How do I Become a Colorado Access Provider?
If you are a new provider joining a group that is already contracted with Colorado Access, please click here to complete a Clinical Staff Update form. If you are a new group or solo provider that is not contracted and would like to become a Colorado Access provider, please e-mail the following information to Provider Network Services at pns@coaccess.com.
- Provider/Practice Name
- Contact name, e-mail and phone number
- Address
- Provider specialty
- Provider Tax ID Number (TIN)
- Provider NPI Number
A representative from Provider Network Services will contact you after we review your information.
